Holidays must be approved by the group leader before they are entered with this form. The group leader has a veto period of two weeks.

- Please inform your group leader directly, if you work at home.

- A notification must be completed as from 0.5 days or more of an absence.

* After 5 days of absence we need a medical certificate.

** Please inform the admin office immediately by phone, if you had an accident.
